1.    I give permission to disclose this information to appropriate staff.

2.    I consent to my daughter receiving Panadol or Antihistamine.

3.    Medications for students may be kept at the Sick Bay.

4.    Please note that if an ambulance is called, St John’s will bill parents directly.

5.     Therefore it is the parents responsibility to pay any costs incurred.
